Abstract:The elderly often suffer from more than one disease and/or geriatric syndrome, and these are named as multiple chronic conditions (MCC). Being of high prevalence in older adults, MCC costs more medical resources, and increases the risks of adverse events, disability and mortality. The concept of MCC indicates that the focus of clinical work for elder patients should be transformed from the traditional disease-centered medical model to the geriatrics model, which includes comprehensive assessment and individualized intervention. This review generally described the concept, epidemiology, influences of MCC, as well as the strategies for diagnoses and interventions.
